Planning Ahead
For senior living and elderly care.
Decisions about elder care and senior living are often made as a result of an injury or illness and a decision regarding home care, home healthcare or assisted living service must be made quickly. However, planning for senior living and elder care will allow you to make the best decisions for all involved. Planning ahead for senior living and elder care can alleviate much of the stress and worry about finding the best option for care at home. Our Program
Upon referral of a client we conduct a comprehensive needs assessment. During the initial visit, we access the client’s functional status, social activities, physical capacity and limitations and nutritional needs. The flowing steps are taken to gather the information needed to develop an individually designed CarePlan that will ensure both the health and safety of each client.
1) Mini Mental Status Test: This test will be conducted to determine the current mental status of our client. How capable they are of making executive decisions..i.e. can they remember what day it is….have they taken their medications…to what degree can they calculate money related transactions.
2) Total Home Assessment: A complete assessment of the clients’ living conditions will be completed in order to determine that they are safe in the home. During this assessment we identify risks factors and make recommendations in the care plan that will help to prevent accidents.
3) Medical Condition Assessment: We complete a medical condition assessment by acquiring a list of past and present diagnosis from their primary care physician as well as a list of current medications and any other vital medical information available. This information is very important in preparing an individualized care plan as well as educating the CareTender that will be placed in the residence to care for your loved one.
THE CAREPLAN
From this assessment, a complete report with findings is generated that includes a recommended Comprehensive Care Plan. This CarePlan is presented to the appropriate decision maker with the goal of implementing a mutually agreed upon CarePlan that will enable the clients to remain safely in their home or residence.
An important part of this plan is as CareManagers, we act as a responsible liaison for long distance relatives, local family members, physicians and other responsible parties. A journal is kept at each residence where we chart activities of interest and pertinent information concerning the clients’ activities of daily living.
The CareTender that is placed in the home is carefully chosen to best fit the needs and personality of the client. Each client is monitored regularly and the CarePlan can be adjusted as needed to provide the very highest quality of Care.
Options
There are so many options available when it comes to elder and senior care. CareTenders, Inc. is available to assist you in developing a care plan tailored to fit your individual needs. Services can be arranged for a few hours a week or as many as 24 hours a day. Short-term respite relief or longer term care is available seven days a week, including holidays.
