Establishing Service
We know how challenging it can be to find the right help for your loved one, and while we’re eager to get started, it’s important for us to take the time to really understand their unique needs. Our process begins with your inquiry, followed by an assessment, and then we create a personalized care plan to ensure we’re providing the best support possible.
By taking these thoughtful steps, we’re able to build strong connections between caregivers and clients, so everyone feels comfortable and supported. It’s all about ensuring safety, comfort, and the best possible care for your loved one.
What to Expect
Initial Assessment
Our intake process for new clients includes
- Mini Mental Status Test: This test helps us understand your loved one’s current mental abilities, such as memory, decision-making, and ability to manage daily tasks like medication or finances.
- Total Home Assessment: We assess the safety of your loved one’s home, identifying any risks and providing recommendations to help keep them safe and prevent accidents.
- Medical Condition Assessment: We gather information on your loved one’s medical history, current conditions, medications, and other relevant details to help us create a care plan that’s tailored to their needs and ensure the caregiver is fully prepared.
Care Plan Preparation
Once we’ve completed the initial intake and assessments, we take the time to carefully create a personalized care plan for your loved one. This plan isn’t just a document; it’s a guide that evolves with your loved one’s changing needs. We’ll meet with the family to go over the plan, making sure everyone is on the same page and comfortable with the next steps.
Ongoing Care
After the care plan is approved, we continue to work closely with you and your family. We act as a liaison between family members, doctors, and anyone else involved in your loved one’s care. We keep detailed notes on your loved one’s activities and progress, so we can make any adjustments needed to provide the best care possible.
CareTenders Who Truly Care
We match your loved one with a CareTender who fits their needs and personality, ensuring they feel comfortable and supported. Our team regularly monitors the care plan and is always ready to make changes to keep your loved one safe, comfortable, and happy at home. We’re here to help every step of the way, so your loved one can continue living in the place they feel most at ease.
Pricing
The initial assessment fee is $250, which includes a comprehensive assessment and the preparation of a personalized care plan. Caregiving and care management costs are discussed at the initial assessment and vary based on individual client needs and level of service.
We operate on a private pay basis, meaning we do not accept insurance. This allows us to focus solely on the quality of care we provide without external constraints.
